was revising some questions on UW... here's one that i don't seem too satisfied with.i want to know wat u ppl think.... Man kicked a ladder ina swimming pool... ended up with open wound 2x4 in first interdigital space x-ray : oblique diaphyseal fracture of 2nd met bone......mild angular dispalcement and open angle laterally vessels,tendons nerves intact vitals stable......most app next step in mx: 1.closure 2.dressing 3.closure and plaster cast 4.dressing and plaster cast 5.open reduction and closure ................well.they say ans is 4 cuz u shouldn't do a primary closure...rather a delayed primary closure as the chances of infection r high???.......... my question is rn't chances of infection high with the wound open like that???.......anyone wants to shed any light.confused???

i think a lot depends on how long ago this man kicked the ladder.if i'm not mistaken-if it's less that 6(or maybe 4-i'm not sure)hours ago-you can close it.and if it's more that 6 hours-you have to do delayed primary closure.and the only condition-the wound is NOT contaminated!
