I wonder how people decide which State Medical Board to choose for the Step 3 without knowing where they have matched. Any insights would be grealy appreciated!
If I take Step 3 in California, can I use the license to practise in East Coast States?

You decide according to what each state is asking you for.
Some states ask you to be a resident to apply, some do not care about that and you can apply without experience. some take a long time, some take less...the thing is that no matter where you apply to do the test ( let's say CA) once you get your permit you can schedule your test on another state (let's say Colorado) and with the results you can apply to residencies in any other state (let's say Ohio and Texas).
So find out according to your situation what is best for you. I cannot give you more info, but read well the materials from the USMLE people and ask here about your specific case...
