hi dear digitaldoc, some new cases are there in cs exam..they are (1) followup visit for HTN with arthritis...and (2)DM with HTN, history s/o stroke.

can u please tell me how to take the history in detail and the dd's, followup tests and counceling...please give me in detail..like in UW...if possible please keep all the councelings for all cases in ur website...it will help so much..as we are asking friends and they are answering only few.for new img's it is very difficult..so please keep all the councelings and new cases in ur website.THANK U SO MUCHHHHHHHHHHHHHHHHHHHHHHHHHHHHHHHHHHH.

Do you remember why we sent this test? – To obtain history
1. Bad News? – Now Reveal the lab findings to the SP and explain the diagnosis in layman terms -
2. Relieve Anxiety - Tell the SP it's nothing too serious and explain how trichomonas is contracted and is treatable.
3. History about

Vaginal Discharge (Amount, color, odor, consistency, blood?),

Sexual H/O - detail

Gyn / OB history – when did you last have a PAP smear done? Or Have you ever ……?
Associated - History pertaining to other STDs (rash, genital lesions, lymph-nodes, etc.)
4. Ask her if she has any new complaints from last time and tell her you would like to repeat a general physical exam.

5. Ask her if she has any specific questions and if she knows how to avoid Trichomonas in future. I would advise you to get your partner/ husband since I need to treat both so that you don’t get it again.

6. Counseling:

a. Explain that both the SP and her partner would need treatment.
b. Tell her that if she will put on Metronidazole therapy - alcohol is to be avoided, since that will cause nausea and vomiting (Disulfiram like reaction)
c. Abstinence from sexual intercourse until therapy is complete
d. Counsel on protection, risk of other STDs like HIV & risk of cervical cancer (need for PAP smears) .
e. Ask her if she has had a pregnancy test recently and when - if not done, ask her to consider having that done. (And mention that on your PN as an Investigation)
f. If not pregnant, advise about birth-control methods.

i am confused.many people say houston is not the place for IMG's to give the exam.what is your opinion?should i change my centre to philadelphia or stick to houston.your reply will be highly appreciated

hi digital doc...i have a prob..applied for cs exam waiting for scheduling permit..have my resident visa expiring on 1st april so have to leave bfore that and going to dallas texas...houston first date available is 17th april 2007 and i have an option to give the exam in chicago but the first date available is 6th june...i want to take the exam as early and come back and study for step 2ck..i want to ask u,people say that chicago and phil are the best centers for imgs....and here in hyderabad kaplan many with over 90 in step 1 have flunked in houston....u have so much of experience...just guide me in a detailed manner....and ur site isnt working either please paste the proper link

i failed at both centers which you mentioned best for IMG. but my friend failed in LA, cleared in chicago. already passed step1 and ck first attempt

can't figure out what went wrong. knocking, introduction, washing hands, helping pt./ offered water, well draping, exam. was o.k, followed pamhugsfoss,asked concern, shake hands,say good bye only thing is counseling-i only said:some cases told the diagnosis, answer their concerns, i'm going to run some test, blood invest, x-ray, ct,or mri, endoscopy, will discuss after getting report.

no money now, if ecfmg return my money, i won't think of usmle.

Indeed, luck does seem to play a big role in Step 2 CS, the scoring system is not transparent and its kinda wierd that non-medicos (the SPs) decide a lot of your fate on the test

digitaldoc i wouldl ike to ask u a question. what if we are asking questions about the CC and the sp says no to each one; do we stop there or continue to focus on it??

also what if in a case you were told baby eating too much or too less; i meant below 95%. how do you approach this kind of CC; please help me clarify this. really appreaciate it.

Dear Digital doc,
I saw your website.please Dix-hallipike maneaveur is not to be done on test and you need specific bed for it. The step2cs is for Generalists we are not otolaryngologist on test.
I dont know wy have you put it there

hi, usmle u, same thing with me didnt do cosure for 4 cases, and no counselling for one case even though iam preparing my self for failue , iam feeling sad, did my exam 4 days ago
