IT depends almost entirely on you ... if you really want this, and are willing to work at it, prepare a great application, advocate for yourself, then you can do it. However, I would recommend you get some clinical experience this year to remind yourself of all of the medicine you've forgotten over all these years. But... if you want it, it's still possible.
