Yes B1/B2 visitor visa is fine to take step 2cs. As long as you have completed your step 2cs by the end of Dec, it should be processed in time for the match. You apply for the visa as soon as you match - some applicants match outside of the NRMP; if so, you should apply as soon as you know where you're going to be training. The only requirement of the J1 home residency requirement is that you leave the US - there is no mandate to my knowledge that you actually have to work in your home country. Clearly before entering a country like Australia, you'd also have to apply for their training programs and visas etc. You're always better to be on a H1b if your employing hospital will take you on this visa class.
