Only under exceptional circumstances. Only rarely does someone drop from one program creating a space. Often residency programs will fill it with a spouse or partner of one of their current residents. Because you are under contract at the hospital that you start at, leaving early could get you into legal hassles, and leave the other residents having to take your call. In all, not a good idea. CGR

Yes it is possible. There are cases where people have to move somplace else, and they just have to leave, there are cases where people realize that's not what they want to do and switch to another specialty. I have two friends who did it, and while researching the residency programs, (you know how some programs have lists of their current residents, and some of them even say where the people went to med school and where did they do their PGY1), I have come across other cases, too. or if the hospital files for bankruptcy and the programme closes as has happened in the past

As noted above others have definitely done it. But I would not take a position at one hospital wtih the hope of reapplying and 'upgrading' the following year. Your chances are poor, you'll offend your program director (who will have to know about your application) and your colleagues. Just go for the best program you can! CGR
