A 60 yr old man presents to a physician coz of difficulty in reading and coming down stairs, which he attributes to en inability to look down. Physical examination reveals that the patient looks around by moving his head rather than his eyes and also shows a distinctive axial rigidity of the neck, trunk, and proximal limb muscles. He shows poverty of movement and dysarthic speech. Mentally the patient responds very slowly but has better memory and intellect than are initially apparent. Which of the following path findings of the brain would most likely be present? 1) Depigmentation of the sunstantia Nigra and Locus ceruleus 2) Diffuse cortical atrophy with relative sparing of primary motor and sensory areas 3) Selective frontal and temporal lobe atrophy 4) Striking degeneration of the caudate nucleus 5) Widespread neuronal loss and gliosis in subcortical sites

confused between 1 and 4! if it were caudate then the pt would hv shown some involuntary movements, so i guess ill go with 1 can easily rule out 2 and 3 (cause loss of intellect), for 5, i think occurs in long term uncontrolled HTN! plz explain as well!

5) Wide spread neuronal loss and gliosis in subcortical sites. the dz is Supra nuclear palsy. Its degenerative disorder. 1 will be more for Parkinsonism 2 Cortical atrophy more of Alzheimers 3 frontal and temporal lobe for Pick's 4 Caudate nucleus for Huntington's. But do refer book too.
