4 yr old. f/up of ear pain. you saw her 4 days ago, prescribed OM and prescribedx amoxicillin 40-60mg/kg/day. now she is no better. exam same as before, next step

A. change to broad spectrum abx

2. continue amox for 4 more days and reevaluate

3. CBC and blood culture

4. prescribe antihistamine/ decongestant

If the child is not improving... change Anx

If the child is worse on exam... then reconsider your diagnosis. the child may have developed meningitis.
