4mt old has swelling on his head. mother explains that the babysitter told her that the baby fell of an upholstered armchair. baby is fussy, irritable and wants to feed continuouslu. O/E diffuse swelling on lf occipitoparietal area. x ray shows 3-4 non depressed linear fractures over lf occiput. next step

a. aptt, pt, plt count

2. ca and P

3. CBC, plt

4. child protective service consultation

5. skeletal survey

There is enough evidence already to suggest child abuse. A 4 yr old cannot sustain trauma of that magnitude.

I chose 5 too and got it wrong. :P

HI all

but surprisingly many review books advice to do the SKELETAL SURVEY first after admitting the child,then inform the CHILD PROTECTIVE ORG.

yes Rashmi, in fact I think that if you read this question in another book or other qbank (other than the one from which merrk got this question) the answer would be effectively SKELETAL SURVEY. I think this is the best next step to follow.

I agree. Its confusing. If you suspect child abuse then complete your exam, go for sleletal survey etc, but if you have very strong suspicion eg several depressed skull fractures which def indicate child abuse, then call the protective services immediately.
