In a Doctor Office 26 years old man, hispanic is complaining diarrhea, abdominal pain and fatigue. His taking Ibuprofen in his regular doses for Arthritis & couple day ago his finished an AB for tonsillitis. The physical exam only found right lower quadrant tenderness. B/P 120/80mmgh & T:100.3 F. Now his recalling to have stool blood 1 weeks ago & has been loss 3 pounds in the last month. Stool blood culture was negative, AFB was negative & Endoscopy biopsy found hypertrophic lymphoid follicles, lymphedema, acute and chronic inflammatory cells and several granulomas. The clinical finding of this patient is more like to be:

A- Ulcerative Colitis.

B- Regional Enteritis.

C- Intestinal tuberculosis.

D- Irritable bowel syndrome.

answer A, because extraintestinal manifestation of ulcerative colitis common as arthritis,spondylitis,primary sclerosing cholangitis,erythema nodosum,pyoderma gangrenosum.

The correct answer is B

The final diagnosis of inflamatory diseases is done by Endoscopy & Biopsy.

In this case the biopsy result give us a big clue. SEVERAL GRANULOMAS. that is characteristics of Regional Enteristis. Ulcerative colitis normally do not give that pathological finding.

Intestinal tuberculosis is the othe pathology that give granulomas, but always AFB is positive. In this case FBA is negative.

Pardon my ignorance, but is regional enteritis the same thing as Crohn's?

regional enteritis is a second name of crohn's disease bacause sharply demarcated areas get affected with skip areas in between.
