A 68yr old diabetic patient has come for routine follow up exam. His Hb A1C is 7.1

His stool for occult blood shows:

negative

negative

positive

what is the next step in management

A. colonoscopy

B. repeat 3 samples of stool occult blood

C. Increase his metformin from 850mg TID to 1000mg TID

D. review his diet and exercise and blood glucose levels at home and re-evaluate at next visit

Ans is A. even one positive test is enough for colonoscopy. Dtool positive for occult blood in a pt > 50yrs is Cancer until proven otherwise!

It is not necessary to repeat 3 more samples.

HbA1c <7 is normal. But 7.1 is nothing to get crazy about.

Isn't this person a candidate for colonoscopic screening just because of his age? Every ten years?
