21 year old man presents with a recent suicide attempt. You are asked to assess him in the emergency room.Which one of the following statements concerning the suicide assessment of this patient is true ?

1 Discussing suicide with persons suspected of suicidal may increase their risk of suicide.

2 Persons with a recent suicide are at less risk for eventually killing themselves

3 Depressed persons who commit suicide can do so as they begin to improve .

4 Persons with schezophrenia are less risk for suicide.

5 Persons with schizophrenia do not usually communicate their intensions

ans is 3. as they are coming out of depression is when they are the most prone.

not only that:

what do we usually give to depressed ones?-right,antidepressants.

1.the first effect of antidepressants is increasing energy level /2 wks/,and then only-antidepressive effect.so they can kill themselves during this "window".

2.that's why you are not supposed to give antidepressants to depresses patient WITH suicidal thoughts-it fact,it's considered malpractice-start with other drugs first.

the first symptoms to improve wiht anti depressants are behaviour eg sleep, appetite, concentration, so the pt may be more likely to commit and to succeed in his plan. Mood and thoughts improve later wiht anti depressants. you may think the pt is getting better b.c he is eatng and sleeping better but his mood hasnt improved yet so he needs close supervsion.

alenka if its consifered malpractice what do you start wiht???

depression and suicide: atmit the pt,

ECT is an option

well,on behavioral science lecture Steven Daugherty said that if you give the depressed patient a prescription for antidepressant but don't assess his suicidal ideation and patient killes himself-it's malpractice.

but for the purpose of the TEST you are supposed to answer SSRIs as the first line of treatment-Alina Gonzales,psychiatry highlights.

yup... I agree. See when you ask about suicide and the patient is currently suicidal... then you MUST admit the patient, even if its agianst his will.

If you give him and SSRi... he may not come back for a follow-up. hehe
