Before the menstrual period women have their premenstrual period (it would be very good if they wouldn´t have it  ), so their breasts are engorged and with mastalgia.

the exact pathogenesis is unknown, but some of the most accepted are precisely those you said (estrogen/Progest levels, increase of PGs).
