Someone in Canada does your statement of need. Although if you aren't a Canadian (Canadien) citizen, you might need to get it from your home country.

Canadian dept of health restricts statement of need to specialties that are in shortage in Canada. At the moment, psych and family practice are available, but many others are not. You may be able to get a statement of need from your own country if you're not Canadian
