Initial test to confirm pneaumothorax

Should it be Expiratory cxr or upright cxr?

USMLE world says upright and Kaplan says Expiratory...appears confusing....please share ur views on it.

pneumothorax is a clinical diagnosis. confirm (if needed)with upright CXR

Either one. Ideally and expiratory upright. Upright puts the air at the apices and lateral spaces so it's easier to detect on an AP or PA. Expiration decreases the lung volume, thereby, 1. slightly increases the ptx to lung vol. ratio so the ptx is better visualized and 2. lung will increase in density a little bit so there is more contrast between the lung parenchyma and ptx.

peace out.
