Sorry.

The correct answer is "F"

A- The enzymatic reaction catalyzed by phosphorylase contain a cofactor pyridoxal phosphatase.

Remember: A property of phosphorylase is that it contains pyridoxal phosphate as a cofactor.

C- The activator factor are cAMP & Ca(2+) located in muscle.
Remember: All pathway for glucose metabolism are interrelated. The end products of the first pathway activate the second pathway based in their need. In this cases the end product of Glycogenesis in muscle is cAMP + Ca2 serve as activator for glycogenolysis.

D- Decrease Insulin, Increase glucagon in liver & increase epinephrine are the final hormonal effects.

Glycogenolysis dicrease the hormonal effect made for glycogenesis. Glucose degradation induce decrease Insulin & Increase glucagon & epinephrine. Remember always exist a ratio insulin/glucagon one decrease the other and viceversa.
