A 70-year-old man comes to the physician because of urinary hesitancy and frequency for 9 months. His temperature is 37.5 C (99.5 F). Examination shows a circumcised penis with no urethral discharge. Testicular examination shows no abnormalities. Rectal examination shows an enlarged rubbery prostate that is nontender to palpation. Urinalysis shows many leukocytes and no erythrocytes. Gram's stain of urine shows gram-negative rods. Which of the following is the most likely cause of this patient's condition? A ) Infection of the epididymis B ) Infection of the prostate C ) Infection of the urethra D ) Neurogenic bladder E ) Outflow obstruction of the bladder

Is this chronic prostatitis?

E. usually due to compression not infection

i go with E. the chronic nature of the syntoms make me think of an outflow obstuction due to BPH

an infection of the prostate will give some tenderness when palpated. also infection is less like without fever or some sistemic syntoms

But why urine examination is abnormal ????Though Significant bcteriuria i not mentioned.E in this case can be the best choice.

yeah, how to explain WBC in UA? and his temp is not normal. in chronic prostitis, there is nontender prostate on rectal exam but it can be rubbery or boggy. please explain

In chronic prostitis, there may be no symptoms, so go to B infection of prostate

Any one can clarify this Q ?

common complications of BPH include urinary stasis, stones and infection due to prolonged urinary retention

(e) outfloww obstruction of the bladder

because with this condition predisposes you to UTI too

chronic prostatitis is painful on palpation.
bacterial--leukocytes and bac
abacterial--leuk + but no bac
prostadynia---leuk - and bac -

All these 3 have pain on palpation.
