if it says weakly gram negative, penicillin resistant, erythromycin sensitive, outbreak in military facility which one do you pick? mycoplasma does not have cell wall, and both are erythromycin sensitive.

tough choice.. well if in the hx it says younger pop i would pick mycoplasma if it has (+) diarrhea, i would pick Legionella just an opinion..

Outbreak in military barrack so may be younger pop, not sure. No diarrhea mentioned though.

look for atypical findings with legionella! like, proteinuria, microscopic hematuria, hyponatremia!

none of that was mentioned. Just pneumonia and that it was penicllin resistant, erythromycin sensitive and in a military barrack.

It can't be Mycoplasma, as it won't grow on conventional media and can't be stained with Gram's stain. http://www.patient.co.uk/showdoc/40000605/ Check out this page for a comparison and differential diagnosis....

I was leaning towards legionella too because of the weak gm stain. Mycoplasma would not stain if it does not have a cell wall. Thanks guys!

I quote from FA 2005: "Mycoplasma pneumoniae: Frequent outbreaks in molitary recruits and prison" page 182. I think its really hard to tell if the questions does not give you more info.
