A 5-year-old Hispanic boy was taken to a psychiatric clinic by his foster mother because of his increasingly disturbed behavior at home. Patient himself said, 'I want to die. My mother died and I want to go where my mother is.' About 8 months earlier, patient's mother died of AIDS. He had a chance to visit her in the hospital shortly before her death. He had not known she was ill and to him her death was sudden and unexpected. He began to experience unremitting hypersomnia, headaches and stomachaches. He had frequent visions of his mother asking him to die and join her in heaven. He became convinced that his body was filled with worms. About 2 weeks before he was taken in for evaluation, he made threats against her foster mother and threatened to kill his 6-year-old foster brother with knife.
The most likely diagnosis is

behaviuoral changes seems reactive, can't make a clear distinction b/w c and d, but i think i am leaning more towards d!

schizophrenia vs maj depression..
how abt the onset of symptoms?
From the q was it 8 mos PTC or symptoms just occurred 2 weeks PTC???
if it's since his mother died 8 mos ago then dx would be schizophrenia, if just 2 weeks ago then Maj depression...
Just a thought bec as we all know onset of sxs very impt in dxng Schizo..

look out for some psychosocial precipitating factor!
as in this case, death of mother!
if it were less than a month, it shuld have been breif psychotic disorder rather than schizophrenia

Childhood schizophrenia is not a dx which you should put in a rush. The symptoms should be present more than 6 months. + when the history is of a grief involvement. You should emphasize on the that side more. Options C, D and E. Out of these 3 the duration of the grief is quite prolonged so it is D.
