Hi bbb, I am a FMG graduate of 09/2003. I have 252(99) and 249(99) on step 1 and 2. I do not have any US clinical experience and can not get any US LORs. I am looking for a good residency program in Internal Medicine which will not decrease my chances for getting into competitive fellowships. I am also interested in H1B visa. I have been calling residency programs but it seems that most of the descent ones require US clinical experience.Does that mean that my only chance is in small community programs?

There are lots of programs out there that hire IMGs without USCE. You just have to search around. My program is on - every year we hire IMGs who have never touched a patient in the USA. And although we are a community hospital, we aren't small. And a small community program? Why are you overlooking the small community hospitals? What's wrong with them? I worked in two different small community hospitals - one urban and one suburban - they produce doctors every year that I would love to take care of me and my family. And they get into fellowships (I'll admit that it's harder, but it can be done) How much do you want to practice medicine? Are you overlooking being a pediatrian, family medicine, or internal medicine doctor? The bottom line is this question: How badly do you want to practice medicine in the USA? How hard are you willing to work to find these programs? Good luck. p.s. If I sound like I am angry - it's probably because I've read this same question many years now on several boards. It takes work and perserverance. No one said that it was going to be easy to practice medicine. It never will be - you'll be studying for the rest of your careers (which I hope are long, happy and healthy)

hi ! bingo! me too in the same boat. I am a 2005 grad. score on step 1 is 267/99. hopefully will get a equally good score on step 2 CK [275/99 maybe??]. But then I got no USCE, or a US LoR or any research experience; Although i have graduated from a highly reputed Indian university with a good rank. I wish to enter a reputed Internal Med program.... TO BBB and Others,... 1] An open ended question: what would you do in my situation? 2] Assume that my scores are 267/99 on step 1 and 275/99 on step 2 CK, do any of these highly reputed programs make any exceptions by waiving off the need for USCE etc. 3] Can I enter into a relatively mediocre program in the 2006 match and then possibly apply for a PGY2 position in the leading Univs in 2007? The point is the program completion rate of these top notch programs are very high and slots for PGY2 positions may be very few.... 4]Finally, what are the damage control measures? Is there any possibility that I might get atleast a couple of months of USCE to show for the match in 2006? How do i go about this? 5]Is doing a transitional year or a 1 yr preliminary a viable solution? If yes how do I go about this ?? I beleive a lot of IMGs will good scores will have these questions and would appreciate replies to the above mentioned Qs in some detail.... Thx....

I have the same Question!! I got a 99 in step one and on the route for hopefully the same score in step 2 in one month. graduated in september 2003. I'm a US citizen. I'm aiming for a top notch IM residency but I'm denied of this since i have no US clinical experience. I've been considering applying for a preliminary or transitional year residency. what are the pros and cons? I'm even willing to apply for a PGY1 position after finishing preliminary. obviously this is what programs that want 3-6 months of hands on cinical experience require( do one year of residency and then apply! ) your input is highly appreciated

Hi everyone I was wondering how easy it is to change your program during residency? Can you go to a categorical IM residency program and after a year reapply for a PGY1 in a top program or no? Is it going to help ypu to get into a top IM program or you are making a bad reputation by leaving your current program?

Nope - it's not going to help you get into another program. Program Directors have to write you a LOR before you will be interviewed at another program, it's a requirement that they do this. If you interview here for a PGY 1 and you have already completed a PGY 1, the program director is going to want to know why you are changing ships. If you are in a preliminary position, it's a bit easier because you haven't given a three year committment. It's all wrapped up in a nice little package called work ethic. Good luck!
