since viral infection accounts for most cases, why do we still give antibiotics routinely?

Many Upper Resp Infections are viral (as you say), some are self limited, so only supportive treatment. But others get a superimposed bacterial infection, so some physicians give antibiotics. I donīt know an exact answer to your question.

According to BP cases in PEDS:"30%of OM is due to viruses,70%due to bact.MCC: S pneu,followed by H inf,Moraxella. " "Uncomplicated OM is usu treated w 7-10 day amoxicillin,bec safe,cheap,good coverage against S.pneu.For recurrent/resistant OM -> change in medic,ear tubes,prophy.abtics...In many countries OM is not treated with abtics,because many cases resolve spontaneously." "Cx.s: mastoiditis,perfor.of tymp. mem.,chronic ear infec.,RARELY: meningitis,brain abscess, lateral sinus thrombosis."
