whats somogyri,.dawn..and waning insulin phenomena are

somogyi and dawn phenomenon are associated with morning hyperglycemia due to hypoinsulinemia(dawn) and with insulin....

how hypoinsulinemia can cause hyperglycemia its very well understood...

somogyi phenomenon explains early morning hyperglycemia occurs due insulin...... insulin causes hypoglycemia which in turn causes a counter regulatory hormone response that produces hyperglycemia....counter regulatory hormones are...glucagon,epinephrine,cortisol....

why this is important is ..cause pt may increase the dose of evening insulin exacerbating the problem....

Insufficient or waning insulin is simple. If the
effective duration of intermediate or long acting insulin ends sometime
during the night, the relative level of circulating insulin will be too low,
and your blood sugars will rise.

Dawn phenomenon refers to increased glucose production and insulin resistance
brought on by the release of counterregulatory hormones in the early morning
hours near waking.

to sort out whether the effect is due to somogyi or dawn phenomenon,person checks his blood glucose at 2 a.m. to 3 a.m in the morning...

if the blood glucose at that time is low then morning hyperglycemia is suspected due to somogyi...

and if the blood glucose is normal or high at that time then it is due to dawn phenomenon....
