A tumor is beginning to compress the right side of the spinal cord at the C3 spinal cord segment and is impinging on the corticospinal and spinothalamic tracts. Wht might you expect to observe first in this patient?
a- Alterations of pain and temperature sensations from the left upper limb
b- Alterations of pain and temperature sensations from the left lower limb
c- Spastic paralysis of the left upper limb
d- Spastic paralysis of the left lower limb
e- Bilateral loss of pain and temperature sensations from the sacral dermatomes

I thought it should be respiratory distress because of Phrenic(C3 )involvement.

well alpha motor neurons are quite deep within the substance of spinal cord , and therefore any external compression will hv other manisfestations first!

I also think it's b. remember piramid decussation, plus spinothalamic and corticospinal transmitt pain and temp. sensations.

very good, it is b!
well remember the fact that as pain fibers travelling into the spinothalamic tract are crossed second order neurons and everynew incoming fiber from the successive segments of the cord places the fibers of lower segments laterally, therefore fibers from sacral segments are more lateral {superficial position} than the fibers from thoracic or cervical segments within the spinoithalamic tract!
significance: extraspinal tumors will affect the lower limbs first and then further impingement will involve progressivley higher segments!
Also, spinothalamic tracts will be invoilved earlier than the corticospinal tracts!
