hi doctors...

in kaplan IM, it was written for treatment of supraventricular tachycardias....do the vagal maneuvar and then...adenosine.....then Ca channel blokers

however while doing usmleworld when i clicked best option of vagal maneuvers..it said that answer is wrong coz vagal methods are not done now....

similarly...kaplan said about giving Ca channel blockers in atrial arrthymias...but usmleworld said...they r not given in atrial arrthymias...

and another confusion....in ventricular arrthymias....which is first drug of choice..amiodarone or lidocain...coz in one book i read first drug is amiodarone...and n second it was written give lidocaine immediately...

can anyone clear my confusions abt cardiac arrthymias mangement....

Regarding PSVT, Vagal maneuvers are still done first, then adenosine...Ca block, B-block...

Atrial fib/flutter, as you know, treatment depends on duration, hemodynamic status, with this in mind, Beta blockers, Ca ch blockers, can be used if safe...

For VT hemodynamically stable, and wide complex SVT's amiodarone is preferred over lidocaine according to current recommendations.

this is what i found in my reading.

in pediatric patient atrial tachy can often be of reentry origin, namely WPW...there you stray away from Ca Ch Block.
