Direct hernias : emerges through the posterior wall of the inguinal canal MEDIAL to the inferior epigastric veins Indirect hernias: passes through deep ring LATERAL to the inferior epigastric vessels, courses through the inguinal canal. Both hernias pass through the superficial inguinal ring.

Only indirect inguinal hernia passes through the deep inguinal ring.

Direct is reducible. indirect is more painful. direct is more common?

inferior epigastric vessle is the surgical hallmark to distinguish 'em _ pathology: direct hernia fr the weakness of the abdo wall so will see in elderly, indirect : fr deep inguinal ring to surperficial ring.... common in younger _ both of them are reducible if not, strangle hernia will happen_ urgent surgery; However this emergency is of indirect , very rare for direct _ painful is sign of compression vessles, nerve, gut.... main reason of hospitalisation, it is indirectophilic
