my Qs is this if a sp asks for a pain medication what should we do in which cases we should say that "i cant give u any pain medication now " and in which case we should offer them pain medication. my thinking is following is this right??? for acute abdomen. no pain medication for chest pain no plzzzz reply it. for cough, body ache pain medication for common cold pain medication for chronic pain ??? for headache ??? .

Just tell the patient " I understand that you are in pain, but I need to ask you a few questions and perform a quick exam to better understand what's going on, then I will definitely give you something for your pain( or other problems)."

thanx, but thats i know .i just wanted to know in which cases we can say in enclosing encounter even before before the test results come that " yes in meanwhile the test results come u can take pain medications " and in which cases even in the closing encounter we would say that "before giving any medication we have to wait the test reults"

This question will be raised only during or right after the history taking, there is no confirmed dx., so there is no medication for unknown reason pain. This question is used to test your interpersonal communication, rather than to ask you do something unlikely. That is my understanding.

yes u r right . but have u seen in USMW case 5 WITH RECENT ONSET COUGH in its checklist in counselling section it is given that we can offer sp acetaminophen.so i just wan to know that what other cases we can give a pateint pain medication before test result. coz one of my friend just gave csa she told me that in one case pateint asked for pain medication and she refused and explaind her but sp was not happy.

I do not know what kind of case your friend met, but one thing I am pretty sure is that without final Dx., no pain medication for abdominal pain pt., it might obscure the Dx. Of course, for pt. with trauma, advanced malignancy, and maybe some others you should not be hesitate to prescribe pain killer for pt. I mean it depends on what kind of cases.

I know I might encounter pain with abdominal pains, but I have no idea whether we will have chances to see pts with severe diseases requring immediate pain management. Everybody's opinions are welcome.

yep in abdom pain it is definite we have to offer nothing but i think common cold , trauma patient some joint pain we can give

Agree, first we can say, we need to exam and (or) run some tests, if sp insis we can offer some for pts.

yes thats wot i think ..we have to convince the patient for a complete physical exam take down a detailed history , do counselling that we gonna wait for the test results until starting any medication and if sp insists for a pain med prescribe something like tylenol until the test results are back and then only will be able to prescribe something more extensive .
