another Q..do i need to give step 3 for getting a H1-B visa...

You will need step three to get an H-1b visa

Should you do observership? - it can't hurt, might help, hard to find.

LORs - mixture of medical school, observership (if you have one) and current professors

PhD? Depends on what field you are applying for.

Different programs require different things. For example, my program doesn't require US clinical experience at all.

Good luck!wink

PhD helps at research heavy programs ... most-university programs. You definitely will need a letter from your PI, but you should still give letters about your clinical skill. Try and do some clinical work and get letters from that, but some programs (MGH for e.g.) will accept time in a PhD program as US experience ... most won't though.

True, Retro..... especially in a community or university affiliated program - they are looking for clinicians and not research persons (although it doesn't hurt).
