Start at the FSMB website - you will have to choose the state you want to register with, e.g. conneticut, texas.. Then fill in the online form and pay the fee. Print off the ID form (or any other forms that are needed) and get it notarised. Once they recieve the notarized form (plus any others depending on the state) you will be sent a scheduling permit. Once you have this you can schedule your test with prometric.

I applied to the Connecticut board-It took 3-4 weeks to get the permit (after I sent them the notraized document) and 4 weeks to get the score.
