The uses of Loop diuretics are: 1... Fluid overload states... The Edemas weather systemic as in cirrhosis or nephrotic syndrome or pulmonary as in CHF (acute pulmonary edema) ...because they act on the ascending limb of henle's loop where 25% of the Na+ filtered load is reabsorbed they are potent drugs and preferably shld be used in the serious ones like acute pulmonary edema 2...Acute hypercalcemia: the reabsorption of Na+,2Cl-,K+ is accompanied by the paracellular reabsorption of Mg++ and Ca++ so blocking Na+ reabsorption wld interrupt the Ca++ rabsorption accompanying it.. 3... Acute renal failure...Loop diuretics...even in low GFR can remain functional... promote both RBF by vasodilatory property and can promote the clearing of obstructed tubules... used along with Fluids... 4...Hyperkalemia: Loops promote K+ wasting... minor uses.. 1.. Hypertension... because their blood pressure lowering effect is unpredictable ... they shld never be first line agents 2...raised intra-cranial pressure: an osmotic diuretic(mannitol) is the preffered agent
