a mother brings her 14 y daughter to your office because she is concerned about her child's eating patterns. The daughter's nutritional history reveals that she generally eats very little because she says she is not hungry. She occasionally engages in junk food binges with friends. She is often constipated. She excercises regularly. She is 5 ft, 6 in. tall and weighs 108 bl. her menarche was at age 13, but she stopped having periods 4 m ago. She says she has no concerns about her body image, and thinks her mother is exaggerating because everyone in the family is tall and thin. The history and findings are most likely associated with a) anorexia b)depression c)hyperthyroidism d) bulimia e) typical adolescent behavior

The answer is e. Typical adolescent behaviour. a.Anorexia is not an answer since it just implies that. The patient has typically binges of junk and that proves beyond doubt that it is just not Anorexia. b.Depression is charecterised by loss of appetitie , loss of sleep, loss of weight and loss of libido. Depressed mood most of the day, and anhedonia.The patient has none of these, and Depression is not the answer. c.Hyperthyroidism is charecteriesed by resting tachycardia, sweaty palms, anxious look on face, staring , diarrhoea etc. Patient has none of these. d. Bulimia is a eating disorder charecterised by episodes of Binge eating , followed by vomiting the whole thing. Icecream is the preferred food since it easily throws up. They have scars on the back of the hand, and are usually plumpy, have boy friend, and there is association with borderline personality disorder and drug abuse. e.Typical adolescent behaviour includes everything. Anorexia at times , episodic binges of junk food,and then occassional constipation( the junk food is devoid of fibre) and her body image is normal. This is the correct answer.

I think the answer is Anorexia bcoz the most imp part of the history is that the patient is having amenorrhoea since last 4 months which is a common finding in Anorexics and it cannot be expalined by any other diagnosis...What is the answer though???

ana is anorexia. pt is doing regular excercises (usually not present in bulimia) and lost period.

AN can be restrictive type or binge or purge type.. her body wt is <85% of expected fr her height and she hasnt had periods fr > 3 consecutive cycles
