A 30 year old African American woman presents for a routine checkup. Her primary care physician orders a CXR which reveals bilateral hilar lymphadenopathy. What next?

a) CT Chest

b) Biopsy of the lesion

c) Oral Steroids

d) Eye exam

e) Check for metastasis

f) Refer to a pulmonologist

g) Bronchoscopy

Answer is D - Eye exam. This is clearly Sarcoidosis. The one thing that has to be excluded is Uveitis, so Eye exam is the correct answer. Biopsy has to be done ofcourse for confirmatory diagnosis. I chose B - biopsy and got it wrong.

its confusing. First we have to confirm a diagnosis ..... biopsy..... then we have to look for or prevent the complications. Besides in sarcoidosis eye changes occur late!!!!

I still feel the ans should be biopsy, because first confirm the diagnosis.
