Chlamydial infection is sometimes associated with a less purulent, less painful but watery discharge compared to GC. So it is a clinical dx since it is difficult and costly($$) to Dx it in the lab and its treated presumptively.
The Ligase chain reaction will probably be the lab test of the future!

Chlamydia infection is now fairly easy to diagnose. Most large labs now use DNA amplification.

We run about 100 or so a day of these tests. It used to be difficult to diagnose, but not any more. The assay we use (above) combines GC and Chlamydia in one sample.

It is important to collect columnar epithelial cells with the swabs provided in these kits, as that is where the Chlamydia live, not in the WBC's or purulent discharge. Indeed, the kit recommends removal of this material so that the swab can get into the male urethra or columnar epis in the female uterine opening. Separate kits are provided for each sex, as they are anatomically suited. The above kit is NOT cleared for eye cultures on infants, however.
