19 yo college freshman comes to the clinic for a 17 pound weight loss. Over the past 5 months, she has increasing difficulty concentrating on her school work and has begun to miss classes. She avoids friends and often skips meals while watching tv alone in her room. She sleeps fitfully and often awakens at 4am and is unable to go back to sleep. her weight is in the low-normal range. She agrees that something is wrong and feels guilty about her academic performance. Diagnosis??? 1- bulimia nervosa 2- schizophrenia 3- conversion disorder 4- obsessive complusive disorder 5- anorexia nervosa 6- major depressive disorder

I dont have the answer to this but I think its anorexia nervosa also.......but why cant cant conversion disorder be the ans?? does anyone know??

6- major depressive disorder its not anorexia nervosa because the patient normally does not accept that they have a disorder. but this girl sees that something is wrong with her and she feels guilty about it. its not conversion disorder because in this type of disoder there is a lack of concern and the patients says "i guess i have to live with it."

I go with major depressive disorder. As far as anorexia- food is only mentioned once in this vignette although she lost 17 pounds.(She is not saying shes fat or overweight) As for conversion in this case there is no evidence of a stressor.

i think major depressive disorder. because does a patient of anorexia nervosa ever sleep fitfully.

Major depressive Disorder: She has Sleep problems She feels guilty, hard time concentrating, depressed because she is avoiding her freinds and school. its been going on for 5 months. Major Depressive Disorder u need all those symptoms for 2-3 weeks.

Answer is major depression definitely! OCD .. no bec. hx. did not mention any rituals.
