It helps a lot to have an US LOR. It isn't a requirement though.
Technicality of application for IMGs makes it difficult to change anything after it is complete.
1. You submit the whole package to the ECFMG, which scans it and transfers to ERAS.
2. Individual programs download your documents from ERAS after you submit your application to them.
After this point it is very difficult to change anything. You can still try to submit new LORs to ECFMG and then call the programs you applied for to download your new LOR.
Or you can just fax them the LOR directly.
