20 yr old in ER for severe headache and fever for last 3 days. CT scan of head shows ring-enhancing lesion in frontal lobe and air-fluid level in frontal sinus. What is the next best step in management?

a) Ceftriaxone alone
b) Cefuroxime + Metronidazole
c) Metronidazole alone
d) Ampicillin alone
e) Ampicillin + Metronidazole
f) Ceftazidime alone

This is classic frontal lobe cerebral abscess. Mostly originating from paranasal sinuses. MC organisms are Streptococcus and Anerobes. So answer is b - cefuroxime + metronidazole.
