how will u diagnose a case of CLL?

>60 y/o pt with painless generalized LAP or
asymptomatic pt with elevated WBC count
diff --80-98% lymphocytes
PBS--smudge cells
BM <20% blasts(replaced with small lymphocytes)

to confirm the dx --> peripheral bld flow cytometry--CD19+ve (or coexpression of CD 19 and CD5) B cells

is there a role of lymphnode biopsy?

what is LAP?

LAP is leukocyte alkaline phosphatase and is present in granulocytes..
malignant granulocytes lack it
its increased in leukemoid reaction and decreased in CML(though 75-95% cells are granulocytes)
CLL is malignancy of naive B cells so, i dont think l node bx wud help in the dx..

thanks mash, i came across a Q in UW about CLL and i picked BM biopsy and they said its the wrong answer as the tap will be dry!!! :?: and that in CLL node biopsy is always the correct answer!! :icon_scratch:

i think the option that said that tap will bdry would have been about FNAC, not LN biopsy as there is no tap in LN biopsy

lymph node biopsy is indicated if lymph nodes begin to enlarge rapidly in a patient with known CLL to assess the possibility of transformation to a high-grade lymphoma.
