A 78 y/o woman with mild renal insufficiency c/o pain in rt knee that has interfered with her day to day activities. Pain is relievd by rest.There are no infl signs of redness/ swelling and minimal jt effusion+.
Xray knee shows osteophytes and asymmetric loss of jt space. ESR and WBC count r normal. The best initial mgmt in this pt is
A) NSAIDS
B)Intrarticular corticosteroids
C)Acetaminophen
D) Total arthroplasty

won't chronic use of acetaminophen or NSAIDs in a pt with mild renal insufficiency affect the pt's renal function?

yes, i was thinking about that. perhaps other choice would be intra artiicular steroids

with mild renal insufficiency too intial treatment is acetaminophen but NSAIDS are contraindicated.acetaaminophen is toxic to kidney only if there is poor liver func otherwise chances in presence of good liver func is only 1%of renal failure.hope that helps :roll:
