3y old girl takes acetaminophen tablets one hr ago.total 36 tablets remaining from bottle of 100 each 325mg and was not full bottle to begin with...child is crying and diaphoretic and wants to throw up.pulse 112/min and rr 24/min.
next step in mgt?
1.detrose & nalaxone
2.n acetyl cysteine iv
3.n acetyl orally
4.supportive and measure levels after 3 hrs

plz explain

toxic level of acetaminophen occur with ingestion of more than or equal to 150mg/kg in children (and >7.0 gms in adults)
ingestion of large amts leads to decreasd absorption frm GIT
so, we can wait fr 4hrs to see toxic levels(becoz by that time most of it is absorbed)

wt of a 3 yr old roughly is 14 kgs...then toxic dose is 2.1 gms
so, we can give activated charcoal and supportive tx till then.

but if we certainly know that the ingested dose is very high and acetaminophen levels wont be available fr 8 hrs then we can straightaway start with NAC
or if the time elapsed frm ingestion is >3hrs measure the levels n proceed accordingly.
