One more question sorry, will the letter from a doctor, whom i'm working with, be usefull as a LOR? This doctor works for one of the Boston medical center as well. Many thanks again in advance.

I am an IMG, scored 89/220 step2,hoping for higher in step1.My dilemma is I don't have US clinical experience.I want to ask the house how to approach this area. Whom to contact and for what capacity eg extenship/observership/research assistantship? And if it is essential to have this experience in order to get into residency program in IM?

No, having US experience is not essential for all IM programs. Some may require it, some won't.
Whom to contact and for what capacity eg extenship/observership/research assistantship?
It's contacting each program and asking if they have internship or externship positions. It's a PITA, but there is no central spot for this information. Good luck
