After nearly drowning an 80 year old man suffers from severe hypoxia with severe brain edema. This edema leads to increased intracranial pressure. When pressure exceeds 200mmHg , the temporal lobe begins to be compressed against the tentorium cerebelli . What is typically the first sign of such compression ? 1. Anisocoria contralateral to compression 2. anisocoria ipsilateral to compression 3. both pupils 5-6mm dilated and fixed 4. external ocular ophthalmoplegia
