Usually due to left subclavian artery obstruction proximal to vertebral a.To get blood to arm blood is "stolen" from vertebralbasilar system. Blood flows backward from vertebral system to distal subclaavian instead of forward to brainstem. Sx-syncope,vertigo,ataxia,dysarthria and upper extremityclaudication. Rx with bypass
