I am having my cs exam on 13 apr .i have some questions if somebody could help me. 1. if the pt complains of pain on superficial palpation of abd should we do deep palpation,cva tenderness,psoas test ,murphys test or just mention the tests to him ,as we re not supposed to repeat painful maneurs.2 .one case from usmleworld where a man with terminal stage of cancer comes for analgesics,should we ask sexual history in this case.please reply me.

As for the first one, you should do Murphy, psoas and cva, but I wouldn't do deep palpation if there is tenderness on superficial palp, even if you would do that on a real patient. In fact, the instructional video on the usmle website tells you not to do "unnecessarily deep" palpation, so I personally did superficial palpation all the time. I don't know if I passed, though. :-)

As for terminal cancer patients and taking a sexual history, gee, I don't know. I guess that depends on your definition of "terminal". All the patients I've heard about on CS have been ambulatory, stable and would be fairly likely to show up in a primary care setting. In such cases, taking a sexual history would be appropriate.
