51y woman with RUQ pain with fever(101),nausea and vomitting.takes no medication and no significant past medical history.murphy sign positive.in emergency dept fever increases to 102F what is next step in management? 1.next day surgery 2.iv fluids and observation 3.iv antibiotic and observation 4.urgent perctaneous drainage

its acute cholecystitis, IV fluids and antibiotics are indicated but u won't observe the patient and early cholecystectomy ..within 72 hrs.. is prefered. So my answer is #1. next day surgery.

pt is in aute state and waiting for 24 hrs cant be next step in management.u cant operate before giving antibotics as the chances of contamination is veryhigh during surgery.Ofcourse fluid ia required too if vitals are scary .
