Early in February, the wife of a 39-year-old male patient calls your office to discuss her husband with you. She is very concerned about him. She states that for the last three and a half weeks, her husband has refused to leave the house for any reason, eat, or shower. He sleeps all of the time and seems to lack interest in anything. She feels that he is depressed and needs help. He does not have a fever or any symptoms of a physical illness. As far as you know, this behavior has never happened with this patient before. He is an accountant and usually an active member of his community. His wife states that he usually has ‘energy to spare’ and is always willing to lend anyone a helping hand. This is also an old Qs...... Answer is D.... i have no explanation for this..... Any one can explain this plz????

I would still go for my answer B. Flu w/out any hx. of fever, no mention of exposure to persons w/ flu, no phy ailment either. Major depression could present like the vignette, plus wife mentioned depression & also start is 2 weeks onward...

Yes the answer is B. It best fits the STEM of the question. It is important NOT TO MAKE ASSUMPTIONS on these questions, UNLESS THEY MENTION THEM. For example, they did not mention it is FLU season or that he has been around anybody else that has the FLU. Dysthemia is a less severe form of depression but is present for about 1 year.

ans C Bc in past he was energetic ,ready to help any one that was Manic phase of bipolar affective disorder and now he is in depressive phase

To ashfaque He USUALLY has ‘energy to spare’ rather than SOMETIMES. That characteristic is not described in episodic manner, which means the pattern is a part of his personality. So you can rule out the possibility of manic episode for that reason.

I GO FOR B. DURATION IS MORE THAN 2 WEEKS. DOESNT SEEM CYCLICAL.NO PREVIOUS HISTORY AS WELL. ALTHOUGHT PT IS HAVING INCREASED SLEEP....
