Usually the result of a demyelinating plaque. Most commonly seen in M.S. medial rectus paresis on lateral gaze. monocular horizontal nystagmus in abducting the eyeand intact convvergence ( this diagnostically important)

:icon_study: As mjl said it's most common in multiple sclerosis. There is damage/demyelination of the MLF, which connects the abducent CNVI with the oculomotor CNIII nucleus. The Patient cannot adduct the eye on attempted lateral conjugate gaze and has nystagmus in the abducting eye. Convergence remains normal.
