ok guys.............here is the answer.......it's easy........ isolation and culture of the organism from blood is the most specific test for lyme's disease............. that was what i thought is correct.......... if anyone has other opinion then plz let the forum know about it...........

Well shirish , the correct answer, and the most specific test is Western blot test detecting IgM and IgG antibodies in the serum.......... But to start with, ELISA/IFA showing abs to borrelia burgdorferi is good enuf..........and when they r equivocal/positive, u SHUD CONFIRM it with western blot.......... So, in general, SEROLOGY IS A MUST for lyme's..............

hmmm, coming to such rarities and confusions of a case, blood cultures cud be done which is a rarity, in testing..........but now, an investigational test and a sensitive one, which is DNA by PCR is better over cultures, coz of the difficult nature of culture of B.burgdorferi..........it cud be done in such cases ...............but the clarity of whether there is an active ds. or if it's a remnant of old one/persisting ds., is not promising with this test, i guess, but well, it confirms the PRESENCE of the DISEASE though............. hope iam close to wat u want, cherry, but i wud appreciate the rite answer if any other though...............

pcr for lyme's disease is still in experimental stage......... i would still say that isolation and culture of the organism from the target rash or blood if possible is the most specific test (85-90 % specificity) ..... although it may be not practiced clinically....but it is the most specific test......... and regarding serology and western blot and all that ....they are supportive in addition to clinical diagnosis....they are not specific for lyme's on their own .....like the culture and isolation...... if you have done kaplan then it's given in notes......also conrad fischer said in his lectures.....that isolation and culture of organism whenever possible is the most specific of all the test for lyme's...........
