A 70 yr old male having osteoarthritis undergoes surgery for removal of his prostate (TURP);during surgery he starts bleeding and it does'nt stop. Lab shows normal platelet count and normal PT and PTT .

what do u think is the cause of his excess bleeding and how u gonna manage it?

Ristocetin assay is normal ; in case u are thinking abt VW disease!

transfuse new platellets as number of platelletes are normal but they not able to adhere or non functioal.

exactly!

the pt is hving OA ... probably taking aspirin ......rendering the platelets nonfunctional......best management sticking out tonguelatelet infusion

might sounds funny...when my boss in surgery doing TURP and bleeding starts...he uses to wash with ice cold saline and wait for few min...bleeding uses to subsides..lol :lol:

hey guyz....this is just a theoretical question tht Dr.Goljan mentioned during his lectures.......

q's on step1 may or may not be applicable in practical life....period!
