answer is: plasma HIV RNA level(HIV viral load)
CD4 count tells you about development of opportunistic infections,

HIV RT PCRgrinetection of virus in blood (detection of a recent infection) and to confirm treatment efficacy.It is the most valuable test for monitoring a patient's progress during therapy and determining the chances of progression to AIDS.

CD4 helper count:Staging the disease and to confirm treatment efficacy.
