Is a ventricular septal defect communicating the left ventricle and right atrium. The normal tricuspid valve is more apically displaced than the mitral valve, so the defect is at the base of the tricuspid valve.

Chest radiograph demonstrates prominent right atrium with left ventricular enlargement.

With Doppler studies, high velocity jet can be seen in the right atrium (coming from the defect).
