I also select wound but further thinking is that stap aureus in not normally habitated in skin but in nosal pharynx. what you think?

Staphylococcus aureus (which is usually MRSA - methicillin resistant Staph. aureus) is found in many many post surgical wound cultures. Unfortunately, many of these isolates find their way into blood cultures also. I am frequently assigned to the aerobic/anaerobic bench at our lab, which includes wounds, tissues, and fluids. Staph. aureus is THE NUMBER ONE ISOLATE on this bench. Many of these are now MRSA - the percentage is approaching 55-60% in our lab. Please please remember to WASH YOUR HANDS BETWEEN PATIENTS WHEN EXAMINING WOUNDS. Medical personnel are the prime suspects in passing on these wound infections. MRSA is tracked by infection control nursing staff institutionally. Don't be the doctor assuming that your hands are clean - and how could you POSSIBLY carry this stuff :evil: , because we are all capable of passing on this bug. Please also be aware that the fourth VRSA in the US (vancomycin RESISTANT Staph. aureus) was just confirmed in the state of Michigan. hi bactitech ...thnx for the gr8 info! i have read in my books about this new drug Synercid[Dalfopristin/quinipristin]is some wht investigational at the moment but apperently this drug has some efficacy against VRSA.. Have u interacted any cases where this drug seem to control VRSA,when the bacterial cultures hv to go through the antibiotic sensitivity tests. Regards

I have never seen VRSA (thank goodness). There have only been FOUR isolates in the whole country so far. Therefore, I can't answer your question. Here is the CDC information on the 2002 Michigan isolate - the 2005 Michigan isolate is not on the CDC website yet: http://www.cdc.gov/mmwr/preview/mmwrhtml/mm5126a1... Hopefully this will give you some more information. When we suspect VRE (vancomycin resistant enterococcus), which unfortunately is getting to be more common every day, we set up three additional drugs: Synercid, Chloramphenicol, and Linezolid. These drugs are only reported out IF the isolate turns out to be a VRE. We also test Synercid and Linezolid on request ONLY from infectious disease docs. It is expensive and is a big gun - only to be used by those that know how to use it. Unfortunately, we don't get to know what drug is being used on these patients. We just report out the results into cyber-land, and the docs take it from there. :|
